There are many different forms of support that can enable you to remain in your home for as long as possible ranging from equipment to people who can come in to support you in your own home. Starting with the Wellbeing Service below, there are many ways in which you can help yourself if you are living at home.

 

 How can the Wellbeing Service help you remain at home?

The Wellbeing Service is available to anyone aged 18 years and over providing interventions which promote independence. Elements of the service will handhold individuals and carers to access other services that they may be eligible for.

Many elements of the service will be available to you free of charge, and some of the service is chargeable.

 

Assessment

The person and assessor will work together to identify the intended outcomes and how they may be achieved.

The assessment may result in receiving a service directly from the Wellbeing Service, or it may be the Wellbeing Service supports access to specialised services.

 

Generic Support

A brief period of support to achieve person centred outcomes. It is preventative by nature and will focus on promoting independence.

Examples of support below:

(not an exhaustive list)

- Support to gain home management and lifestyle skills.

- Access to befriending services.

- Supporting access to various support activities provided by local clubs, voluntary or community organisations.

- Support where a person requires support/advice to remain in their own home.

- Support to secure and maintain appropriate housing.

- Help in managing finances and benefit claims.

- Help in completing forms to enable them to stay independent in their own home.

- Supporting individuals to understand the options available by gathering information and explaining in a way that makes sense.

 

Access to Simple Aids for Daily Living

- Supply and installation of low cost equipment such as key safes, raised toilet seats, bath and shower seats and chair raisers.

 

Minor Adaptations

- Undertake minor adaptations including supply and fitting of grab rails, hand rails, banister rails and alterations to steps.

 

Telecare Services

- Installation of Telecare equipment linked to a 24 hour monitoring centre, helping you to live independently at

 home but safe in the knowledge that help can be summoned quickly when needed. This may be from a family member, friend, the Wellbeing Response service (subscription required) or if necessary, the emergency services.

- Monitoring of the Telecare equipment through a range of sensors that can be worn (for example, a pendant round your neck), or placed in your home.

The sensors can alert the monitoring centre 24 hours a day. The system works through a base unit connecting you to the monitoring centre via your phone line.

 

 Wellbeing Response - Stay Safe

- 24 hour 7 day a week responder service promoting confidence to remain independent at home. Responding to Telecare alarms by attending a person's property for non-medical emergencies.

 

Home from Hospital - Home Safe

- Home Safe is a transport and resettling service for individuals returning home from a hospital stay. Our drivers and support staff will take you home where you will be met by a Wellbeing Response team member, who will help you settle back in.

Lincolnshire Independent Living Partnership provides the Wellbeing Service in West Lindsey, Lincoln, South Kesteven, South Holland and Boston.

 

East Lindsey District Council provides the Wellbeing Service in East Lindsey.
